About The Position

The Triage Nurse provides comprehensive triage guidance to patients of all ages across the clinical services areas, including Family Medicine, Women's Health, and Communicable Disease (CD). The nurse is responsible for assessing CHA patients' conditions-by telephone or in person- to determine the urgency of medical needs and to guide patients to the appropriate level of care. The Triage Nurse collaborates closely with other healthcare professionals to ensure timely and efficient delivery of medical services.

Requirements

  • Requires graduation from an accredited nursing school obtaining RN or BSN.
  • Must complete the introduction to Public Health Credential course within the first year of employment.
  • Will manage PHN II program by 3 months of employment.
  • Bilingual in English and Spanish required.
  • Experience working with patients of all ages across life span; maternity patients.
  • Outpatient, public health, urgent care or hospital experience required.
  • A valid license to practice as a Registered Nurse in North Carolina.
  • Current CPR certification.
  • Valid NC driver's license.
  • Must have strong assessment, critical thinking, and decision-making skills
  • Possess excellent communication and interpersonal abilities
  • Ability to work effectively under pressure and prioritize competing demands
  • Must be proficient in electronic medical record (EMR) systems and other healthcare technology

Responsibilities

  • Triage patients in all 3 clinical areas (Family Medicine, Women's Health and Communicable Disease) via telephone and/or walk-in using the Clear Triage platform as guidance
  • Document all phone and in-person encounters in the EHR
  • Complete tasks in the EHR including medication refills, lab follow-up, communication to provider, nurses or integrated staff
  • Complete Prior Authorizations
  • Work in conjunction with clinical supervisors in panel management, ER and hospital discharges and closing care gaps
  • Function, as needed, as a back-up to nursing staff in any of the clinical areas to include: vital signs, complete medical history, provide education, assess vaccinations, order labs, diagnostic tests, clinical procedures, administer medications and vaccinations
