About The Position

The Arc Prince George’s County serves more than 600 individuals with disabilities through a variety of programs, services and supports. Our primary areas of support are community living, family and personal supports, employment services, and day programs - all with an emphasis on person-centered community involvement and inclusion. The service coordinator supports children, families and ITP team members to ensure that children referred to the Infants and Toddlers program obtain all the available services and resources they need in a timely and efficient manner. S/he works under a grant that is administered by the Prince George’s County Public School system (PGCPS) and receives daily guidance and direction from PGCPS. ITP service coordinators are assigned to a variety of roles within the program that may include: Intake, Case Management, Clerical, Transition and Family Resource supports. The service coordinator is expected to be a cooperative and supportive member of the inter-disciplinary ITP team and a strong advocate for each child.

Requirements

  • BA or BS Degree highly preferred but not required.
  • However, candidates should have some college hours and at least two years of related experience.
  • The candidate must possess excellent oral and written communication skills, and they should be highly organized with good attention to detail.

Responsibilities

  • Functions as an integral member of the ITP team. Collaborates effectively with all team members.
  • Assists families in accessing services, resources, etc. and provides support as needed.
  • Serves as a resource person for new and potential ITP clients and families.
  • Completes home visits and corresponding assessment of children and family’s strengths, resources and needs.
  • Collaborates with school personnel and family to obtain assessment of child. Arranges transportation and/or interpretation services as needed.
  • Facilitates the delivery of therapeutic services.
  • Follows medical assistance regulations for documentation for ITP targeted case management.
  • Maintains record in approved ITP record format.
  • Attends in-services and other required training to enhance knowledge and skills
  • Includes family, school personnel and other relevant professionals in the IFSP process. Ensures that IFSP forms are correctly completed according to established format.
  • Expected to travel throughout county for meetings
  • Assists in the implementation of the IFSP by participating in related meetings as required.
  • Provides resources, referrals and linkage services to meet the needs of the child and family.
  • Appropriately interacts with families and professionals while in attendance of meetings/workshops.
  • Demonstrates understanding of ITP regulations by providing proper guidance and direction to families.
  • Helps to strengthen families’ own advocacy skills. Has ability to explain public and private systems to families and help them to access these systems.
  • Explores free private and public resources. Uses creativity and advocacy skills in securing resources.
  • Provides supervisor with an accurate itinerary weekly
  • Actively participates in team meetings, facilitating as requested.
  • Completes payroll/timekeeping responsibilities
  • Completes assigned paperwork and documentation in a thorough, timely manner.
  • Performs other duties and tasks, which are within this general occupational category and responsibility level.
  • Provides informal interpreting upon request.
