The Caminos Home Study Post Release Services (HSPRS) Clinician provides community-based intensive case management services to recently migrated children, adolescents, and their families. The Clinician is responsible for applying their clinical expertise in the provision of strengths based, culturally and linguistically appropriate intensive case management services. With a focus on stabilization of the family unit, the Clinician utilizes interventions tailored to the needs of recently migrated youth and their sponsor family, in the community where their sponsor resides. Providing weekly intensive in-home services, the Clinician engages in a continuous clinical assessment process, ensuring safety and stability of the youth and family while building a healthy relationship and rapport. The Clinician supports the delivery of Everstand’s trauma responsive model by developing service plans that incorporate individualized interventions to address needs unique needs of the youth and family, utilizing a range of financial, medical, social, vocational, psychoeducation, crisis stabilization and other services and resources.
