About The Position

Bilingual Health Services Care Manager (Russian) The Health Services Care Manager, within the framework of the Aging Service Access Points (ASAP) regulations, determines program eligibility and provides a consumer centered approach to assessment, service acquisition, reassessment, and monitoring of the services for the state home care and specialized MassHealth funded programs, to assist elders to live independently in the community. Schedule: This is a Local Union 509, full-time, non-exempt (35 hour/week) position. We offer 4-day work week and hybrid work schedule (once training is complete.) No nights or weekends. Compensation: Starting annual salary range is $50,872.46 - $54,804.09. A 6% increase will be used to recognize the required bilingual skill set in Russian that is required for this role. Additional duties may be assigned as required. Top Key Activities: Provide case management to assigned caseload while meeting AGE timelines and documentation standards Develop, authorize, implement and monitor a care plan within budget guidelines to meet identified need(s) during periodic reassessments by telephone, video conferencing, and by visiting the consumer in their home Coordinate community-based services, and ensure access to public benefits with consumers, families, caregivers, and service providers. Provide crisis case management prevention and intervention. Maintain accurate consumer information in case records and consumer services database systems in accordance with the Executive Office of Aging and Independence (AGE), MassHealth and Mystic Valley Elder Services. Other Activities: Cross collaboration with all departments for best practices Participate in an interdisciplinary team model and regular supervision meetings Attend AGE and MVES training sessions On call as needed Key Success Measures: Consumer satisfaction State and MVES quality metrics Qualifications: A Bachelors' degree preferred from an accredited school in Social Work, Psychology, Counseling, Human Development, Nursing, Sociology, Criminal Justice, Public Health, Public Policy, or Gerontology plus at least two years of experience in counseling, casework or case management. Fluent in English and Russian. Requirements

  • A Bachelors' degree preferred from an accredited school in Social Work, Psychology, Counseling, Human Development, Nursing, Sociology, Criminal Justice, Public Health, Public Policy, or Gerontology plus at least two years of experience in counseling, casework or case management.
  • Fluent in English and Russian.
  • Excellent interpersonal skills with demonstrated ability to communicate across all levels, including staff, family and external partners
  • Ability to maintain confidentiality while working with sensitive information or in sensitive situations
  • Highly resourceful team player, with the ability to work effectively independently or as a team
  • P riv ate transportation required
  • Demonstrates a willingness to take on new tasks with a general attitude that no task is too small, impossible, or cannot be improved
  • Expertise in the following platforms: Microsoft 365 (Word, Outlook, Teams, Excel, and PowerPoint), Adobe Pro, Zoom and other web-based applications
