About The Position

At Oak Orchard Health, the Bilingual Community Health Worker (Spanish/English) serves as a vital link between patients, providers, and essential community resources in the Brockport office. This hands-on, community-focused role involves helping individuals navigate the healthcare system, identifying and addressing barriers to care such as transportation, language, literacy, and financial challenges. The position is crucial for improving health outcomes and strengthening communities, particularly by supporting the local agricultural population and connecting families with critical services.

Requirements

  • High school diploma or equivalent
  • Bilingual Spanish/English required (critical to supporting our patient population)
  • Community Health Worker (CHW) certification or willingness to obtain
  • Ability to support a wide range of responsibilities—from patient navigation and scheduling to case management, follow-up, and community engagement
  • Strong interpersonal, communication, and organizational skills
  • Comfort working with diverse populations while maintaining professionalism and boundaries

  • Experience in community health, social services, healthcare, or outreach preferred

Responsibilities

  • Help patients navigate the healthcare system, including scheduling, confirming, and coordinating appointments
  • Conduct needs assessments and identify barriers such as transportation, language, literacy, and financial challenges
  • Provide Spanish interpretation and written translation within your scope
  • Connect patients to community resources like food, housing, insurance, and medication assistance
  • Coordinate transportation and other access-related services
  • Educate patients on health promotion and disease prevention
  • Support outreach efforts and build relationships within the community
  • Collaborate closely with clinical and care teams to ensure coordinated, patient-centered care
