Community Health Worker (Bilingual) - Part Time
About The Position
The Community Health Worker will work primarily with specific target populations including but not limited to non-English speaking populations of geographic target areas. The Community Health Worker Peer will work closely with internal and external stakeholders or partners including but not limited to clinical/medical/dental providers, primary care teams, and other agencies to improve patient care and outcomes. Patient outreach interactions may be telephonic, video/digital, or in-person generally at an office site. Depending on department, Community Health Worker Peers may need to be bilingual or be knowledgeable of specific department terminology such as medical or dental.
Requirements
- Completes and maintains all competencies and trainings as required for role.
- High School Diploma - OR - GED - OR - Relevant experience may be considered in lieu of education
