About The Position

The Care Coordinator is responsible for managing AHN member cases and ensuring members meet their obligations to AHN and its provider network. This role advocates for members, resolves issues, and ensures continuous care and support. The Care Coordinator guides members through the healthcare system, assists with navigating treatment plans, and helps connect them to appropriate resources.

Requirements

• High school diploma required; AA degree preferred.
• Minimum two years of experience working with socioeconomically diverse or disadvantaged populations.
• Completion of the Community Health Worker (CHW) course within 120 days of employment.
• Bilingual in English and Spanish required.
• Must pass a State of Nevada background check.
• Be able to sit at a computer for 6-8 hours per day.
• Be able to stand between 3-6 hours per day.
• Be able to withstand hand/wrist deviation and repetition.

Responsibilities

• Build and maintain positive relationships with AHN partners and providers, demonstrating professionalism and consistent follow‑through.
• Respond to all calls, emails, texts, correspondence, and process client referrals within 48 hours.
• Gather information to estimate surgery costs and communicate scenario options to members.
• Understand CPT/ICD coding, medical billing, and discuss balances with clients.
• Maintain accurate and timely documentation in Salesforce and keep all case records updated.
• Adapt to department-specific schedules and effectively manage multiple priorities.
• Check eligibility and assist with socioeconomic support services, including scheduling, in the following areas:
• Transportation
• Nevada Care Connection
• Respond to inquiries from potential members and employers, offering clear information and timely resolution.
• Conduct program orientations for new members as needed.
• Possess an understanding of available social services, both governmental and private.
• Ensure full compliance with HIPAA regulations.
• Provide front desk coverage when needed.
