The Urgent Crisis Center (UCC) is a behavioral health urgent care center that provides crisis evaluation, de-escalation and community coordination of care for children and youth experiencing a behavioral or psychiatric crisis. All evaluations, assessments, safety planning and initial recommendations for follow-up care must occur within a 24-hour time period. Work in partnership with local hospitals and community agencies is imperative to ensure fluid movement throughout the spectrum of services. All staff are required to work their assigned schedule but may be asked to shift/alter this in response to high workflow time periods. GENERAL STATEMENT OF RESPONSIBILITIES: The Bilingual Crisis Care Coordinator is responsible for direct services with and community outreach to children, adolescents and their families seen within the Urgent Crisis Center. They work primarily to support families in the implementation of their crisis plan and coordination with community providers. They will partner with Crisis Clinicians in crisis assessments and planning in the UCC, and then collaborate with the family in accessing additional resources in their community. The Crisis Care Coordinator will have an assigned schedule, but hours worked may vary according to client need and may include evening, night and weekend support. They may also participate in community outreach efforts to educate referrals sources to UCC services. They maintain confidential records and collaborates with other professionals and team members.
