Summary: The NYS 1115 Program Navigator plays a key role in helping New Yorkers understand, access, and utilize services available through New York State's 1115 Medicaid Waiver program. This position provides outreach, education, enrollment assistance, care coordination support, and linkage to community-based and clinical services that address health-related social needs (HRSN), improve health outcomes, and promote health equity. Essential Duties and Responsibilities include the following. Other duties may be assigned. Conduct community outreach to raise awareness about services available under the NYS 1115 Medicaid Waiver. Engage individuals, families, and community organizations to promote participation in eligible programs. Assist individuals in understanding eligibility criteria and screening potential applicants for the 1115 Waiver program. Support clients by completing enrollment applications and gathering required documentation. Assess clients' needs related to medical, behavioral health, and social care (e.g., housing, food access, transportation) and connect clients to appropriate clinical providers, health plans, and community support. Coordinate follow-ups with clients and/or resources providers to ensure clients receive scheduled services and resolve barriers. Maintain accurate case records and client progress notes in various databases Participate in training and meetings as required Qualifications Education and/or experience required: Bachelor's degree requires 3-5 years of experience in a social service agency providing case management support Skills, Licenses, and/or competencies required: Experience in community outreach, case management, social services, or healthcare navigation. Knowledge of Medicaid, social service systems, or community resources. . Strong communication, interpersonal, and problem-solving skills. Ability to work with diverse populations Proficiency with computers and electronic documentation systems. Familiarity with NYS Medicaid managed care and waiver programs. Experience working with individuals, experiencing housing and food insecurity, behavioral health needs, or chronic conditions. Bilingual required Position Type and Expected Hours of Work: This is a full-time position. Days and hours of work are generally 9:00 AM to 5 PM Monday -Friday. Additional hours may be required to meet program deadlines, or client needs. There may be periods when work hours vary Working conditions and physical demands required: The demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Ability to travel and conduct outreach and various meetings/events throughout the Bronx. Work with clients who require consistent and comprehensive support, some under potentially stressful conditions.
