About The Position

The Independently Licensed Behavioral Health Therapist (LiSW, LPCC, LiSW-S, LPCC-S) provides behavioral health services for Neighborhood Family Practice patients. The Independent BH Therapist can oversee the behavioral health needs of adults, children, adolescents, and their families both in person and via telehealth to support the holistic care of the person.

Requirements

  • Master’s degree in Social Work, Counseling, or similarly related field required.
  • Active and unrestricted License in state of Ohio as Independent Social Worker (LiSW) or Counselor (LPCC) or similar required.
  • 2+ years experience as Social Worker or Counselor required.
  • Able to meet insurance panel enrollment standards required.
  • Demonstrated experience and skills in clinical areas needed to diagnose and treat NFP patients.
  • Able to establish trust and professional relationships with patients of diverse cultural and ethnic backgrounds.
  • Able to work collaboratively within a team of behavioral health professionals.
  • Computer proficiency with EPIC EMR/EHR, Microsoft Outlook Suite, and other clinical applications.
  • Able to effectively manage crisis situations with patients.
  • Able to communicate effectively, both verbally and in writing, including through the use of electronic communication devices.
  • Manual dexterity sufficient to operate a computer keyboard.
  • Able to sit, stand, and stoop for extended periods of time.

Nice To Haves

  • Trainee designation (LiSW-S, LPCC-S) preferred.
  • Experience in Community Health setting preferred.
  • Ability to speak a second language relevant to patient population fluently preferred.

Responsibilities

  • Screen, assess, diagnose, treat, and follow-up with Neighborhood Family Practice patients referred to receive Behavioral Health therapy.
  • Provide individual or group psychotherapy to assigned patients.
  • Work with parents, guardians, spouses, schools, and other support systems to manage cases, providing communication and care coordination.
  • Ensure the timely and thorough completion of all required documentation within Electronic Medical Records, chart records, patient forms, collaborative forms, or other required paperwork.
  • Provide immediate crisis intervention for NFP patients as needed.
  • Collaborate, coordinate, and consult with Neighborhood Family Practice’s professional staff who provide healthcare services including primary and preventative care, midwifery and perinatal support, and other specialty services.
  • Promote the integration of Behavioral Health services by serving as a liaison, resource, and consultant within the department and organization.
  • Assist other Behavioral Health staff with the treatment of patients, both individually and in group settings, including case discussion, collaboration, and planning.
  • Participate in department and assigned Care Team meetings to further the strategic initiatives of the organization.
  • Remain up to date on best practices by attending relevant training courses.
  • Other duties as assigned.
