About The Position

The City of Virginia Beach is seeking a PT Clinician II - Quality Assurance (QA) Specialist to support continuous quality improvement efforts within the Intermediate Care Facility (ICF/IID). The ideal candidate will have experience with regulatory and policy compliance, quality improvement, training and support, and reporting and documentation. The Clinician II would have good working knowledge of Centers for Medicare & Medicaid Services (CMS) and Intermediate Care Facility (ICF) regulations, state licensing requirements and quality assurance standards. Duties: Complete Individual Support Plans (ISPs), nursing notes, staff training, medication administration, and active treatment documentation. Monitor compliance with federal (CMS) and state (DBHDS or equivalent) regulations and facility policies. Ensure confidentiality of resident and employee information in all QA activities. Participate in internal reviews, mock surveys, and quality improvement initiatives. Assist in developing, implementing, and evaluating quality improvement projects. Track and follow up on corrective actions to ensure timely resolution. Collect and analyze data to identify patterns or areas of concern in service delivery. Provide feedback and recommendations to the management team on findings and best practices. Support the development and maintenance of quality assurance tracking tools and dashboards. Support training efforts by identifying staff development needs based on audit results. Provide coaching or technical assistance to staff to improve documentation and compliance. Participate in interdisciplinary team meetings to support alignment between quality goals and resident outcomes. Maintain detailed records of audits, findings, and corrective actions. Prepare clear, concise reports summarizing trends, outcomes, and recommendations for leadership review. Work closely with nursing, QDPP, direct support, and administrative teams to improve service delivery. Foster a culture of accountability, transparency, and continuous improvement.
