About The Position

The Family Medicine Residency Behavioral Health Clinical Coordinator is an integral part of the FM Residency Program providing, clinical support and training to the FM Residents.As part of a multidisciplinary treatment team, The Cl will assist the FM residents through IBH support including warm hand-offs, screening utilizing evidence base tools, assessing, diagnosing and providing brief treatment . The IBH Clinician will develop curriculum for the Residents building skills in a trauma informed, culturally responsive integrated treatment approach to patient care and communication. The Clinician will participate in creating and supporting Resident wellness education and activities. The Clinician has advanced clinical and cultural competency expertise with diverse communities including knowledge of community resources and needs. The BH Clinician will be embedded in the FM care team for 15 hours a week and maintain a clinic schedule of 21(4 admin) hours/week.

Requirements

  • Master’s degree and independent license in Behavioral Health Field (ex. LICSW, LMHC, )
  • Prior experience participating in Graduate Medical Education activities

Responsibilities

  • Provide case consultation and training to FM Residents
  • Develop curriculum and provide training to Residents on Motivational Interviewing, CBT and other topics identified by Residents
  • Provide support and resources for resident wellness
  • Collaborate with Medical Leadership to conduct evaluations of residents integrated care skills
  • Assess clinical emergencies, including risk of suicide and danger to others.
  • Deliver 21 clinical hours of individual, group, couples and or family interventions
  • Applies knowledge of Psychiatric Diagnosis (DSM 5 & ICD-10) to perform and record mental status examination and findings and billing codes with assigned patient caseload.
  • Apply knowledge of methodologies of psychotherapy to provide individual, group, and family behavioral health counseling
  • Provide referrals to appropriate resources and follows-up as necessary.
  • Obtain progress notes from outside providers involved in patient care.
  • Engage and empower patients to understand their health issues and work closely with patients to take proactive measures that will result in positive outcomes.
  • Perform timely and accurate record keeping in accordance with all Health Center requirements.
  • Complete clear, specific, and behaviorally measurable treatment plans with assigned patients.
  • Maintain cooperative relationships with Health Center employees, patients, community agencies and the public.
  • Work in cooperation with co-workers and supervisory staff.
  • Collaborate with team members resolving problems in a constructive manner.
  • Maintain a willingness to listen to and respond to constructive feedback from supervisory personnel.
  • Serve on Health Center committees and workgroups as requested.
  • Perform other duties as assigned for which you are qualified
