About The Position

Good Shepherd Hospice, a member of Catholic Health, promotes the physical, emotional and spiritual well-being both of the person completing life’s journey and their family members, providing comprehensive and compassionate end-of-life care of the highest quality. Good Shepherd helps patients and their families live each day fully, focusing on quality of life, control of symptoms and the things that matter to them the most. The Bereavement Specialist provides emotional support and counseling to family members and significant others of hospice patients following a loss. This role implements bereavement care plans, conducts assessments, and ensures services align with hospice philosophy while supporting families through the grieving process.

Requirements

  • Master’s degree in Social Work, Counseling, Psychology, Mental Health Nursing, or related field.
  • Minimum 2 years of clinical experience, preferably in hospice or bereavement care.
  • Current New York State professional license required.
  • Knowledge of grief counseling and hospice care principles.

Responsibilities

  • Develop and implement bereavement care plans with the interdisciplinary team.
  • Conduct post-death bereavement assessments and provide grief counseling.
  • Facilitate bereavement support groups and coordinate follow-up with families.
  • Provide referrals to community resources and support services.
  • Assist with memorial services and bereavement education for staff and the community.
  • Maintain accurate documentation and collect program data.
