Our dynamic Crisis Response Team is vital in providing high-quality behavioral health services to those who need them most. As a key member of our specialized team, you will conduct critical assessments and care for individuals and families in crisis across Weld County and a 12-county region in Northeast Colorado. This rewarding role includes responding to emergency walk-ins, crisis calls, and after-hours on-call situations. You'll have an opportunity to make an immediate impact by performing thorough evaluations, identifying the best care options in the least restrictive settings, and facilitating connections to suitable resources. In this position, you'll also offer intensive care coordination for Weld County clients and support discharge planning, aiding individuals in successfully transitioning from higher levels of care back into their communities. Behavioral Health Workers support the implementation of individualized care plans for clients facing mental health and/or substance use challenges. They adhere to the policies, rules, regulations, and directives of North Range Behavioral Health while facilitating client engagement. Behavioral Health Workers act as sources of emotional support, assist with resource navigation and problem-solving, and advocate for clients' needs to ensure their active participation in care plans.
