Responsible for assessment, counseling and therapeutic interventions to individuals, families, and groups. The counselor collaborates with primary care providers, psychiatric staff and other multidisciplinary team members to support whole-person care and improve patient outcomes. Principal Accountabilities Standards of Performance: Respect, Integrity, Compassion, Collaboration, Stewardship, Accountability, Quality Education Master’s Degree in Social Work or Counseling Licenses and Certification LCSW or LCPC in Illinois Experience and Skills Technical Experience: 2 years of experience in providing individual, family and/or group therapy required. Strong interpersonal and communication skills In-depth knowledge of community resources Skill in diagnostic formulation (DSM-5) Proficiency with EHR systems and documentation standards Knowledge of trauma-informed and recovery-oriented care. Physical Activities Intermittent hand manipulation required Intermittent lifting and carrying of 20 pounds
