Assesses the patient and prepares an individual Master Treatment Plan and monthly reviews and updates thereafter. Performs mini-mental status exam, psychosocial assessments, facilitates both individual and group therapy, documenting all interactions in a time frame compliant with company, state, and federal guidelines. Is responsible for contacting any patient who is absent for 3 or more days to determine reason for the absence and when they plan to return to the program. Responsibilities Designs a unique Master Treatment Plan for each patient based on diagnosis and issues identified in the assessments. Reviews and updates treatment plans every 30 days. Performs a mini mental status exam and psychosocial assessment on all new patients. Performs other duties as assigned. Provides group and individual therapy and prepares timely documentation in compliance with company, state, and federal guidelines. Performs related duties as required
