The Behavioral Health Targeted Case Manager provides case management and care coordination services for members identified as high risk, having ongoing mental health and/or substance abuse conditions or psychosocial needs to ensure care is appropriate, cost effective, and that appropriate resources have been identified and coordinated. The Behavioral Health Targeted Case Manager assists members in achieving comprehensive health care through assessment of needs, assisting with access to the right care, at the right time and in the right location, working to increase quality standards and outcomes providing opportunities to achieve the highest level of recovery and optimal wellbeing. The Behavioral Health Targeted Case Manager is tasked with providing services to adults who have been diagnosed with a serious and persistent mental illness (SPMI) or children who have been diagnosed with a severe emotional disturbance (SED). Mental health targeted case managers work with patients to assess and address the profound effects of housing instability, food insecurity, and other social determinants of health. Using a person-centered approach with the person served, the case manager assesses, plans, refers, coordinates and assists the person in gaining access to services. Must be self-motivated with the ability to work independently, a problem solver and fiscally responsible. Must be able to assess, analyze, summarize and present report data with effective oral communication skills. Must be able to multitask and work with little direction.
