About The Position

Help clients and their families navigate and access community services, other resources, and adopt healthy behaviors. Will be expected to complete additional trainings, such as motivational interviewing, Narcan training and bloodborne pathogens to be able to add that knowledge for services delivered to clients they will serve.

Requirements

  • High school diploma or GED required
  • Valid driver’s license with access to reliable transportation
  • Non-tobacco user
  • The successful candidate will need to be highly detail oriented, organized, and dependable with demonstrated proficiency in computer, communication, and multi-tasking skills.

Nice To Haves

  • Associates Degree preferred
  • Certified Health Education Specialist (CHES) and/or Certified Family Peer Supporter (CFPS) credential preferred Certified Peer Supporter (CPS)
  • Preferred experience, training, and abilities in the following areas; Substance use education, grant/program management, community engagement, and partnership building.

Responsibilities

  • Identify services and resources (advocacy, education, navigation to services, etc.) needed by individuals and families impacted by the substance use disorder of a family or household member and work with Harm Reduction staff and/or community partners to connect individuals and families to services.
  • Establish positive, trusting relationships with clients and their families while providing general support, encouragement, and feedback.
  • Motivate clients to be active, engaged participants in their health.
  • Follow-up with clients via phone calls, home visits, and visits to other settings where clients can be found.
  • Assist clients with accessing health related services, completing applications and forms, understanding care plans and instructions, utilizing services
  • Give appointment reminders and attend appointments (as approved)
  • Enter data into electronic and hard copy records, spreadsheets, and databases Complete client reporting within two business days.
  • Provide advocacy and assist with navigating services for individuals and families with agencies, organizations, and entities as requested by individuals and families.
  • Collaborate with Harm Reduction staff to ensure a coordinated approach to community substance use and overdose prevention initiatives.
  • Maintain confidentiality of confidential or sensitive records and information pertinent to the assigned position.
  • Driving to community meetings, client visits and trainings.
  • Performs other related duties as assigned or requested
  • Assisting with grant reporting or other grant activities, as assigned.
  • Assist with ZMCHD’s marketing and branding efforts by attending health fairs and special events, preparing displays for events, developing educational materials and working with the ZMCHD PIO and media.
  • Support and participate in quality improvement activities.
  • Adhere to ZMCHD policies and procedures. Practice dependable attendance habits.
  • Represent the department and agency favorably to the public. Adhere to the adopted ZMCHD customer service standards.
  • Ensure compliance with standards, laws, and regulations as promulgated by regulatory agencies such as OSHA, Federal and State government.
  • Ensure compliance and work toward the mission of ZMCHD.
  • Maintain certifications and improve knowledge and skills through professional development opportunities.
