Provides consultative services and actively seeks opportunities to collaborate with the health care team. Recognizes complex learning needs and consistently demonstrates effective patient and staff education. Demonstrates and role models effective communication to coordinate patient and family-centered care. Facilitates appropriate follow up care and/or refers patients (within the designated patient population) to the appropriate next level or site of care. Serves in an advocacy role on behalf of patients, families, staff, and caregivers to ensure safe, equitable healthcare provision. Ensures evidence-based care and continuously monitors patient and program outcomes. Furthers professional growth by engaging in learning activities according to Board of Nursing standards.

Responsibilities

Collaborates with interdisciplinary team, attending physicians, families and other professionals to formulate and initiate specialty norms and maintain collaborative practice agreements.
Utilizes advanced clinical knowledge to provide or guide safe and effective continuum of care.
Collaborates in establishing the plan of care, coordinates and implements care delivery while anticipating patient care and program needs across the continuum.
Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis.
Continually utilizes the nursing process to evaluate patient and program progress towards goals and applies appropriate interventions to ensure effective outcomes.
Employs strategies to promote health and a safe and healthy environment.
