This Work Matters! As the Behavioral Health Provider (BHP), you are a part of the primary care treatment team that identifies, triages, and manages patients with medical and behavioral health needs within the primary care setting. In addition, the Behavioral Health Provider will provide skill training through psycho-education and patient education strategies and will develop specific behavioral change plans for patients and behavioral health protocols for target populations. Working with the care team in situations in which a patient may be experiencing a behavioral health crisis and take the lead to coordinate support and stabilization for the patient as well as continued monitoring of the patient's presenting behavior. The position requires the applicant to be either a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor in the state of Oregon.
