This position supports clinical programs and Medicaid AE patients by providing care navigation for patients with a behavioral health, substance use disorder, or social determinant of health needs. Essential Functions of the Position Coordinates and tracks behavioral health referrals to internal and external behavioral health clinicians and resources. Triages patients identified with social determinates of health (SDOH) or substance use disorder (SUD) needs to coordinate and track referrals to community-based organizations (CBO). Maintains extensive knowledge of community-based organizations, external resource availability, and eligibility guidelines for each. Facilitates transportation eligibility, scheduling, and other logistics for appropriate Medicaid AE patients. Performs triage of notifications from CBOs and alerts appropriate clinical staff. Collects and updates the release of information (ROI) documentation to ensure coordination of patient care between organizations. Participates in interdisciplinary care conferences with primary care providers and other care management teams to develop an individualized plan of care. Assists with clinical administrative support to Brown Health Medical Group Primary Care's Medicaid AE program and point of contact for community-based organizations. Participates in Behavioral Health and AE team meetings. Complies with federal and local confidentiality laws, including HIPAA, ensuring patient privacy. Adheres to Brown Health Medical Group Primary Care's guidelines and policies for protecting patients' demographic, clinical and financial information. Performs other job-related duties as assigned.
