About The Position

THE BEHAVIORAL HEALTH LEAD OVERSEES THE STLC BEHAVIORAL HEALTH PROGRAM AND EMPLOYEES FOR ALL ASSIGNED STLC LOCATIONS. HE/SHE MANAGES PROGRAM IMPLEMENTATION, PROCESS IMPROVEMENT, EXPANSION, AND SUSTAINABILITY. THIS ROLE SERVES AS A CONTACT POINT FOR EXECUTIVE LEADERSHIP AND COMMUNITY PARTNERS. IN ADDITION TO THE LEADERSHIP DUTIES OUTLINED BELOW, THE SELECTED INDIVIDUAL WILL CONTINUE TO ENGAGE IN DIRECT PARTICIPANT CARE WORKING IN COLLABORATION WITH AN INTERDISCIPLINARY CARE TREATMENT TEAM TO FACILITATE THE PROVISION OF HOLISTIC, WHOLE-PERSON BEHAVIORAL HEALTHCARE FOR ALL STLC PARTICIPANTS. THE BEHAVIORAL HEALTH LEAD WILL ALSO CONTINUE TO PROVIDE BEHAVIORAL HEALTH AND SUBSTANCE ABUSE SCREENING, BRIEF INTERVENTION, EDUCATION, AND REFERRAL SERVICES FOR PARTICIPANTS WHERE NEEDED. PROVIDES ONGOING CONSULTATION AND TREATMENT PLANNING SUPPORT FOR BEHAVIORALISTS, NURSE PRACTITIONERS, SOCIAL WORKERS, AND TEACHING PERSONNEL TO MEET THE NEEDS OF INDIVIDUAL PARTICIPANTS AND STLC. WILL WORK WITH PARTICIPANTS WITH PSYCHOSOCIAL, BEHAVIORAL HEALTH, CHRONIC DISEASE, BEHAVIOR CHANGE, AND/OR SUBSTANCE USE CONCERNS. IS KNOWLEDGEABLE AND ADHERES TO ALL POLICIES, PROFESSIONAL PRACTICE STANDARDS, PHILOSOPHIES AND REQUIREMENTS FOR PROFESSIONAL CLINICAL PRACTICE.

Requirements

  • Bachelor's degree, Master of Social Work and post MSW experience, successful completion of required licensing examinations
  • 2+ years of management experience required.
  • At least 1 years experience working with the frail elderly population.
  • LCSW, LCMHC, LMFT. Full license required
  • Meet a standardized set of competencies for the specific position description established by Senior TLC, Inc. and approved by CMS before working independently.
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact
  • Excellent and effective written and oral communication skills.
  • Interacts in a friendly, professional manner with a wide range of patients, operations staff, physicians, other departments, and community partners in a friendly, professional manner.
  • Knowledge of psychosocial experiences of older adults, particularly focused on trauma-informed treatment modalities, and various forms of treatment.
  • Ability to lead and manage an interdisciplinary team.
  • Ability to plan, prioritize, and complete delegated tasks with attention to detail and efficient time management.
  • Ability to work well under pressure and various time constraints.
  • Proficiency in Microsoft Office Outlook, Word, Excel, PowerPoint use and email communication.

Responsibilities

  • Provide programmatic oversight of assigned behavioral health staff within the STLC Sites.
  • Demonstrate and apply knowledge of program policies/procedures to identify issues.
  • Update/maintain accurate and secure patient medical records.
  • Ensure other behaviorist team members are documenting appropriately, completes, in a timely manner, all required documentation related to patient treatment and organizational operations.
  • Identify and solve any process, personnel, or clinical challenges.
  • Regularly update Medical Director and Executive Program Director with changes and/or challenges to school behavioral health services staff.
  • Collaborate with PCPs on referral processes and community partnerships.
  • Ensure the preparation, completion, and submission of required reports.
  • Evaluate program activities to improve quality of care and program efficiency.
  • Actively participates in QI activities, including monitoring utilization of data metrics.
  • Participates in monthly assigned meetings as required.
  • Conduct a monthly meeting with all behavioral health staff
  • Engages in continuing education to maintain licensure and enhance professional competence relevant to medical, substance use and legal issues that impact patient care.
  • Performs other clinical or administrative functions as directed by the Medical Director and Executive Program Director
  • Oversight of performance measures and productivity of the team.
  • Support recruitment, hiring, retention, and training staff.
  • Complete annual competencies within the required time frame

  • Travel between STLC sites
  • Other duties as assigned.
