This position involves coordinating and assisting in the provision of Behavioral Health resources and services to clients. The role requires assisting families through a screening and triage process to determine needed resources and services, answering their questions and concerns, and providing service referrals and scheduling. Key responsibilities include gathering referral and pre-admission information, completing all pre-admission data entry, and identifying emergent or complex cases for clinical review before finalizing referrals. The coordinator will collaborate with interdisciplinary teams to ensure efficient resource and scheduling services and will utilize Electronic Medical Record and Scheduling software to assess service demand and capacity. Maintaining confidentiality of all clinical and administrative information consistent with agency policy is essential.
