Position Overview: The Counselor will: Facilitate patient involvement in treatment and education within the milieu Conduct individual, group, and family counseling sessions Perform utilization review and support complex discharge planning Collaborate with healthcare team members to ensure timely, safe, and resourceful care Interpret and communicate complex patient and family needs, serving as a role model for caring practices Assess psychosocial risk factors by evaluating prior functioning, support systems, and reactions to illness Counsel patients and families regarding emotional, social, and financial impacts of illness/disability Mobilize family and community resources to meet patient needs Conduct mental health assessments upon physician referral Support complex discharge planning and crisis intervention efforts Serve as the point person for cases involving child abuse/neglect, elder abuse, domestic violence, and institutional abuse Act as liaison with child and adult protective services (APS/CPS) and community agencies Provide intervention in guardianship, foster care, adoption, and mental health placements
