Behavioral Health Consultant/Mental Health Counselor Location: West Salem Clinic |1233 Edgewater NW, Salem OR 97304 Job Status: Full-time, Monday - Friday Sign-on Bonus: $3,000 (Internal candidates that are currently employed here at NWHS are not eligible for a sign-on bonus.) Referral Bonus: $1,000.00 (Referral bonus’ expire at the close of this job posting.) YOUR ROLE: In this role you are going to be rotating job functions between a Behavioral Health Consultant (BHC) and a Mental Health Counselor. As a BHC you will work directly with medical patients to break down behavioral barriers and support the medical team by providing consultation and support on medical and behavioral issues. As a Mental Health Counselor, you will provide mental health services, provide on-going assessment, treatment planning, and goal setting. S PECIFIC DUTIES - BHC: Identify patients experiencing negative outcomes with their chronic conditions that could benefit from the use of brief therapy to assist them with behavioral interventions that could improve their outcomes, increase their self-management skills, and reduce behavioral barriers to wellness by proactively scrubbing. Engage on same-day access appointments with patients and consultation with clinic staff. This will includes receiving warm handoff from medical providers related to mental health problems needing brief, evidence-based interventions including anxiety, depression, bipolar spectrum, substance abuse, prescription drug misuse, trauma, thought disorders, cognitive impairment, ADHD in adults and children, and parent training for children with behavior problems. Assess the nature and extent of each patient's emotional, behavioral, MH, and/or COD presenting problem, and provide any brief interventions necessary, typically ranging from 1-5 face to face visits (with some ongoing telephonic outreach) to ensure behavioral barriers to appropriate care are reduced. Demonstrates awareness of co-occurring disorders and chronic conditions and how each impacts patient outcomes and care. SPECIFIC DUTIES – MENTAL HEALTH COUNSELOR: Assess the nature and extent of client's MH and COD presenting problem, formulate a plan for service that addresses MH and COD issues and their impact on client's interaction, and ensure complete and timely documentation pursuant to OAR 309.019.0140. Ensure productivity requirements are met by providing mental health treatment to individuals, couples, families, and groups that improve the client’s level of functioning. Ensure clinical supervision requirements are met pursuant to OAR 833.030.0031.
