About The Position

Will support the care team in identifying and behaviorally intervening with patients to improve their physical health condition Will assist individuals in improving their health status and managing chronic illnesses. will assist with medication adherence, treatment plan adherence, self-management support and goal setting, and facilitate group classes. Interventions for individuals with behavioral health problems. Coaching sessions

Requirements

Ability to communicate clearly and effectively with co-workers and patients.
Ability to complete competency CPR and First Aid
Ability to prioritize and compete multiple tasks simultaneously.
Effective oral and written communication skills.
LCSW, LMSW, LPC's: LMFT, Clinical Psychologists and/or individuals working towards certification and are currently in clinical supervision.
