About The Position

We are looking for a LCSW, LPC, LMFT, or Psychologist to join our team of behavioral health experts! At Philadelphia FIGHT, the Behavioral Health Consultant is embedded in the medical clinic to offer their expertise on a patient's psychosocial stressors and behavioral health issues. Our Behavioral Health Consultants work with medical providers and patients to support whole-person wellness by screening patients for mental health and substance use conditions and referring them to mental health/substance use treatment when appropriate. The ideal candidate has a combination of skills, including case management, assessment, diagnosis, and brief interventions. Must be licensed in the state of Pennsylvania and have 2-3 years of experience to be considered for this role. About the Role: Schedule: Full-time, Monday - Friday (9:00AM - 5:00PM) Location: Philadelphia FIGHT Community Health Centers, Philadelphia, PA 19107 Compensation & Benefits: FSLA status is exempt with an annual rate of $70-85K. 3 weeks of PTO annually + 11 paid holidays 403B with up to 4% employer matching that is fully vested starting day 1 Medical, vision, and dental insurance starting day 1 FSA for medical, dependent care and transportation expenses Discounted degree options with partnered institution Pierce College Student loan repayment options

Requirements

  • Current Pennsylvania license in Clinical Social Work (LCSW) or Psychology (Ph.D. or Psy.D.).
  • Minimum of 2 years of experience as a BHC or 3 years of experience as a therapist required.
  • Excellent interpersonal skills and ability to work collaboratively with medical providers, support staff, and other professionals across multiple disciplines.
  • Competency in providing cognitive and behavioral interventions to individuals, families, couples, and groups with behavioral health/substance abuse problems in a primary care setting.
  • Must possess basic computer skills to perform job duties including desktop computing, electronic medical records, e-mail, time sheet management, reporting, internet research, etc. using relevant software such as HRIS and Microsoft Word, Excel, etc.
  • Discretion, initiative, and ability to maintain HIPAA compliance.

Nice To Haves

  • Prior experience delivering services to adult persons with histories of incarnation, homelessness, mental illness, substance dependence, and trauma preferred.
  • Experience with incarcerated or formerly incarcerated individuals preferred.

Responsibilities

  • Consults with individual patients to screen for mental health/substance use disorders using an evidence-based tool.
  • Consults with Primary Care Providers (PCP) to enhance understanding of the patient; follows up on positive screens to refer patients to substance abuse and/or mental health treatment when appropriate.
  • Provides decision support for treatment planning and assists in the implementation and monitoring of biopsychosocial treatment plans.
  • Provides psychoeducation to patients and their support systems on mitigating risks for substance abuse and mental health disorders and prevention strategies.
  • Utilizes motivational interviewing to create self-care plans and skills training to facilitate disease self-management, improved coping, distress tolerance, stress reduction, and relaxation.
  • Conducts brief problem-solving cognitive interventions to modify negative thinking and promote self-efficacy.
  • Alerts appropriate clinic staff and aid in crisis intervention as needed.
  • Provides consultation and coordinate care of patients with clinic staff.
  • Identifies, refers, and advocates for patients needing intensive behavioral health service, substance abuse treatment, and other services as needed.
  • Documents all activities related to patient care (assessments, progress notes, etc.) appropriately in the Electronic Medical Record and within the established timeframe set forth by FIGHT policy.
  • Completes all billing documentation as needed (billing codes and other documentation) as required by contracted funding sources.
  • Abides by all compliance standards including but not limited to completing training and attending all required team/staff meetings, clinical supervision meetings and/or other check-ins.
  • Upholds and ensures compliance and attention to all FIGHT policies and procedures as well as the overall mission and values of the organization.
  • Other duties as assigned to fulfill FIGHT's mission.
