About The Position

Provides quality comprehensive dually diagnosed assessments, interventions, education, referrals and consultations to and for patients in the primary care setting; in addition to supporting the integrated model of care services by consulting with other providers and educating other staff both clinical and administrative both as needed as requested.

Requirements

Master's degree in the Behavioral Health field (Psychology, Counseling, or Social Work) with at least two (2) years of experience as an independently licensed provider is required.
Licensed in State of Ohio as LISW, LPCC or psychologist.
Ability to work independently as well as a valued member of an integrated team.
Computer experience required; previous experience with EMR preferred.
Ability to work with a culturally diverse group of people.
Must be willing to work flexible hours as required by position.

Responsibilities

Consults and collaborates with primary care providers and other staff regarding all areas of behavioral health related issues for children, adolescents, adults, and families.
Provides brief, solution-focused behavioral health interventions with active development of Behavioral Change planning.
Cultivates education, administration and self-management skills for targeted populations with psychological or behavioral health issues.
Provides education to assist with enhancing primary care providers utilization of behavioral health interventions.
Promotes and supports the integrated model of care that fosters a smooth interface between medical and behavioral health care throughout the organization.
Collaborates and co-manages patients behavioral health care with consistent, expeditious, and clinically significant feedback to referring providers.
Provide crisis triage and intervention on-site as needed.
Manages referrals both internally (received) and externally (initiated).
Attends site, departmental, and organizational meetings as required.
Formulates specific plans of care for patients assessed and diagnosed with mental health, substance use, and other related psychiatric illnesses.
Documents patient assessment/intervention and other data in medical records using established medical record forms and documentation templates (as required by organization); while adhering to regulatory bodies regarding completing documentation.
Provides patient education to individuals, families, and groups about health maintenance and management of chronic disease process and behavioral health related conditions.
Effectively communicates facts, ideas, and skill that change knowledge, attitudes, values, beliefs, behaviors, and practices for individuals and families.
Establishes an interpersonal relationship with individuals or families intended to increase or enhance their capacity for self-care and coping.
Participates as a member of a multidisciplinary team to facilitate teamwork, improve patient care processes and outcomes.
Provides services and interventions in accordance with organization policies, procedures, and protocols.
Demonstrates communication skills that are clear, effective, facilitate patient care and team functioning, and is respectful of differences.
Establishes and maintains professional working relationships with other members of the organization overall.
Maintains professional competence by pursuing opportunities for continuous learning, attending continuing education offerings, reading professional literature, and engaging in other activities, which develop new knowledge and skills.
Maintains knowledge of, and complies with, personnel, behavioral health, medical and clerical policies and procedures.
Supports site and organizational programs, projects, and initiative as required.
Supports other behavioral health providers and staff, medical providers and staff, and corporate and other administrative to honor patient care and the overall needs of the organization.
Performs other related tasks as needed to ensure continuity of care
Performs other duties as assigned.
