Summary Statement The incumbent in this position serves as the primary advocate for individuals on their caseload, with responsibilities that include developing and monitoring person-centered plans, maintaining regular communication with service providers, families, and guardians, and conducting routine in-person visits at provider site locations. The incumbent is also responsible for ensuring all services are delivered in compliance with applicable standards and regulations for each individual's service plan. Routine travel to provider site locations is a requirement of this position as well as the occasional need to provide emergency transportation for individuals.
