About The Position

EDUCATION, LICENSE, CERTIFICATION and EXPERIENCE: 1. Education: Has completed bachelor’s degree or higher in behavioral health field (i.e. social work, psychology) 2. Licenses: None required 3. Certifications: None required 4. Experience: Experience with children and adolescents or older adults preferred REQUIRED SCREENINGS: Drug Screen Physical Assessment Tuberculosis screen Background check (Criminal, Civil, Educational, Previous Employment, etc.) Driver Record screen (positions requiring on-the-job driving) ESSENTIAL FUNCTIONS: Reliable and punctual attendance is essential; expected to be at job as scheduled each day. Conduct thorough assessments of physical, mental, social, and environmental needs. Create individualized plans with goals, involving clients, families, and providers. Link clients to services (medical, housing, vocational, etc.) and ensure cohesive care across providers. Provide immediate support for clients in acute distress. Track client progress, adjust plans, and ensure quality of care. Champion clients' access to resources and rights. Maintain accurate, timely, and complete clinical documentation, including assessments, progress notes, treatment plans, and billing codes. Return calls and messages promptly (within 24 hours of receipt or sooner if clinically necessary). Teach clients and their families about behavioral health conditions and recovery strategies. Adhere to relevant laws, policies, and ethical standards related to behavioral health services. Prepare and submit assessments, progress notes, reports, and summaries to entities/agencies (funders, insurance companies, etc.) as required. Participate in meetings, quality improvement activities, and committees as requested. Other job related duties as assigned. GENERAL DUTIES: Requires confidentiality regarding patient information / records. Requires confidentiality regarding employee information. Requires confidentiality regarding business processes. General office equipment (copier, fax, printer, electronic health record, etc.) Wage starts at $31.00 per hour and goes up with experience
