About The Position

Clarvida is seeking a Behavioral Health Care Coordinator to support children, adolescents, and families through community-based behavioral health services. In this role, you will provide case management support, coordinate care, and serve as an advocate by connecting clients to essential resources and services that promote stability, growth, and improved outcomes. This is a hands-on, client-facing position that requires strong communication skills, flexibility, and the ability to work collaboratively with treatment teams in a community-based environment.

Requirements

Bachelor’s degree in a Human Services discipline from an accredited college or university
Experience working with children and/or adolescents in a therapeutic, community-based setting
CPR and First Aid certification (or willingness to obtain prior to working with clients)
Strong organizational, communication, and teamwork skills
Ability to work flexible schedules and travel between service locations

Responsibilities

Assist in the case management of children and adolescents
Actively support clients and families in achieving treatment and service goals
Collaborate with multidisciplinary treatment teams to develop and evaluate care plans
Serve as an advocate and resource connector for individuals and families
Coordinate services and referrals to community organizations
Maintain accurate, timely, and compliant documentation
Work flexible hours and across multiple locations as required by program needs
