About The Position

The Behavioral Health Care Coordinator – Team Lead (BHCC Team Lead) is responsible for the oversight of the team of Behavioral Health Care Coordinators in the delivery of quality patient care in collaboration with a multidisciplinary team. The BHCC Team Lead will support an integrated care approach in all clinical services. The BHCC Team Lead is responsible for high quality supervision for all staff assigned and is also responsible for providing direct clinical care for a minimum of 25% of their assignment with the expectation that all BHCC shifts are covered. The BHCC Team Lead must be capable of independent action without losing sight of the fact that their actions always represent the policies, priorities, and values of Axis Health System. Must be knowledgeable regarding patient health information and HIPAA compliance. Must be organized and demonstrate the ability to follow guidelines. Must demonstrate excellent interpersonal and communication skills. Must demonstrate the ability to be self-directed. Must demonstrate the ability to follow the organization’s confidentiality and security rules.

Requirements

  • HS diploma, GED or higher
  • EMT-B, NREMT certification and have three years’ experience in a medical or related setting
  • Knowledge of mental health medications and mental health diagnoses preferred.
  • Knowledge of diverse issues including mental illness, co-occurring disorders, substance abuse, TBI, dementia and developmental disabilities.
  • Knowledge of community mental/physical health resources and their eligibility criteria and provide rapid linkage to treatment providers.
  • Ability to deliver services in various locations.
  • Communicate confidential information only as required to perform job responsibilities. Working knowledge of HIPAA and 42 CFR, Pt 2.
  • Working knowledge of involuntary mental health holds (2CCR 502-1.27-65), and alcohol and drug Emergency Commitments (EC) and Involuntary Commitments (IC).
  • Ability to follow established universal precautions and report potential exposure to a communicable disease immediately.
  • Knowledge and ability to operate standard office equipment including scanner, computer, calculator, copy machine, fax, and multi-line telephone.
  • Working knowledge of Windows based computer applications including MS Word, MS Excel, MS Outlook, and ability to access the internet.
  • Ability to be helpful and sensitive to the needs of all employees, while concurrently respecting corporate policies, procedures, and priorities.
  • Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers.
  • Ability to communicate job-related information to individuals and small groups.
  • Ability to work independently with minimal supervision; plan own work schedule effectively and multi-task and prioritize in a fast-paced setting.
  • Ability to multi-task, organize, concentrate and prioritize workload in a sometimes-hectic environment with frequent interruptions.
  • Ability to read and understand written materials.
  • Must regard work as confidential.
  • Must have current driver’s license, qualify for commercial vehicle insurance, and maintain a driving record that allows insurability with AHS’ insurance agency.
  • Travel required
  • Ability to occasionally work beyond normally scheduled hours.
  • Ability to take on additional duties when necessary.
