About The Position

Wood River Health is committed to delivering compassionate, team-based care.  We are seeking a compassionate, organized Behavioral Health Advocate to join our team-based, patient-centered care environment. In this role, you’ll support patients with behavioral health and substance use needs, help reduce barriers to care, and work closely with behavioral health clinicians, primary care teams, and community partners.  The Behavioral Health Advocate plays a deeply meaningful role in helping patients overcome barriers, feel supported, and stay connected to the care they deserve. Requirements

  • High School Diploma or equivalent required.
  • Positive disposition, strong organizational abilities, and a commitment to community health.
  • Strong written and verbal communication skills.
  • Ability to multitask, adapt to interruptions, and manage changing schedules.
  • Cultural sensitivity and ability to work with diverse patients and staff.

Nice To Haves

  • Bachelor’s degree in Psychology, Social Work, or Human Services preferred.
  • Prior experience or an internship in a medical or behavioral health setting—especially within an integrated or case management model—is preferred.

Responsibilities

  • Assign behavioral care programs to new patients and provide outreach to new and returning patients to help reduce barriers to care.
  • Use department protocols to schedule and support patients in keeping appointments.
  • Assist in engaging “at-risk” patients who may need additional encouragement or support.
  • Provide patients with approved self-management and educational materials to support healthier decisions and improved outcomes.
  • Work closely with Care Management and Community Resources to assist patients needing additional services.
  • Manage incoming and outgoing referrals efficiently and accurately.
  • Participate in daily medical team huddles when high-priority patients are scheduled.
  • Consult with primary care providers to facilitate administrative warm handoffs.
  • Attend internal Behavioral Health meetings and represent Wood River Health at designated community-based organization meetings.
  • Assist with department organization and help maintain a smooth workflow.
  • Schedule patient appointments and handle rescheduling due to provider changes.
  • Maintain current department procedures and resource materials on SharePoint in partnership with IT.
  • Document all patient interactions in the electronic health record promptly and thoroughly, following approved processes and templates.
  • Demonstrate a customer-centered approach through respectful interactions and supportive communication.
  • Uphold strict confidentiality and follow all HIPAA regulations.
  • Support and embody Wood River Health’s mission, vision, and values in all work.
  • Adhere to organizational policies, procedures, and compliance standards.
  • Perform additional duties as needed to support patient care and team goals.
