As a Behavioral Health Advocate, you will be responsible for case management and utilization review of behavioral health and substance abuse cases. You'll have a direct impact on the lives of our members as you recommend and manage the appropriate level of care throughout the entire treatment plan. Expect to spend about 50%25 of your time in the field visiting our members. You’ll need to be flexible, adaptable and, above all, patient in all types of situations. If you are located in Central Maryland (Columbia, MD or surrounding areas), you will enjoy the flexibility to work in both the field as well as remotely as you take on some tough challenges. Requirements

  • One of the following current and unrestricted licenses to practice in the state of Maryland: LBSW, LMSW, LCSW or RN with 2+ years of behavioral health experience
  • 2+ years of behavioral health experience
  • Intermediate level of proficiency Microsoft skills (Word, Excel, Outlook)
  • Ability to travel 50%25 and have reliable transportation

Nice To Haves

  • Outpatient Case Management experience
  • Dual diagnosis experience with mental health and substance abuse
  • Experience working in an environment that requires coordination of benefits and utilization of multiple groups and resources for patients
  • Experience working with maternal population

Responsibilities

  • Addressing social determinants of health and connecting with appropriate resources
  • Make patient assessments in home setting and determining appropriate levels of care for a case load of members
  • Obtain information from providers on outpatient requests for treatment
  • Determine if additional clinical treatment sessions are needed
  • Administer benefits and review treatment plans and coordinate with the IDT
  • Coordinate benefits and transitions between various areas of care
  • Identify ways to add value to treatment plans and consulting with facility staff or outpatient care providers on those ideas
