About The Position

Responsible for providing home/community-based case management and/or behavioral support services to clients and their families receiving evidence-based services such as Dialectical Behavior Therapy. Advocates for the clients and families to access appropriate and necessary resources from Human Services and similar departments/agencies to improve the quality of their lives. Requirements

  • Bachelor’s degree in a human services field required
  • Skilled in operating various medical record software and hardware, word-processing, and database software programs
  • Flexible schedule with some evenings
  • Excellent multitasking and communication skills a must
  • Ability to work independently and part of a team
  • Current driver’s license, reliable transportation, registration and auto insurance
  • Ability to communicate effectively
  • Travel to and from clients’ residence, community locations and office site, which could include using walkways, stairs and/or elevators
  • Ability to lift up to 20lbs
  • Must be able to work remotely and in person adhering to PHI requirements

Nice To Haves

  • Spanish Speaking preferred and is compensated
  • Bilingual skills are compensated by an additional 6%, above base pay.
