Directs the development, coordination and administration of policies, services and programs that optimize operational functions to improve clinical outcomes and financial performance in Continuing Care. Collaborates with leadership to identify best practices, develop operational strategies and cost effective solutions. Participates in the development and implementation of financial, operational and strategic planning activities. Facilitates quality improvement initiatives and cross-business unit collaboration. Work with external partners, ensures, governmental bodies and community based organizations to advance the care of at-risk patients.
