About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency operating throughout Arizona, providing residential, outpatient, inpatient, physical health, medication-assisted treatment, and crisis services to individuals experiencing substance use disorder, homelessness, and mental illness. CBI is a non-profit provider utilizing cutting-edge, nationally recognized, evidence-based models. The Associate Director of Housing and Community Integration is responsible for overseeing outreach, shelter, housing, and community-based programs within their designated area. This role involves direct supervision of management-level staff, ensuring program compliance with contract requirements, meeting productivity targets, and achieving defined program outcomes. The position also focuses on the development of assigned managers and staff, including creating training plans to address gaps in care. Primary responsibilities include collaborating with managers on innovative service delivery approaches and guiding their implementation, partnering with the CBI finance department for grant management and budget tracking, and monitoring overall program compliance. Additionally, the Associate Director will represent Community Bridges within the community, seeking new partnerships and funding opportunities to expand the continuum of care and improve the system. This role will also serve as the liaison for CBI's Patient Centered Medical Homes, Crisis, and Inpatient units to streamline connections with Housing and Community Integration resources and knowledge.

Requirements

  • Highschool diploma or GED required
  • 1 year of experience in a behavioral health position providing outreach and engagement activities required
  • Must have 1-3 years of grant writing experience, program development and/or grant monitoring
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
  • Current AZ Driver's License (valid and in good standing)
  • Clear 39-month Motor Vehicle Record (MVR)

Nice To Haves

  • Associate’s degree or higher in a field related to behavioral health preferred
  • Minimum of six months of recovery from substance use and/or mental health disorders preferred
  • Peer Certification (if applicable)

Responsibilities

  • Overseeing the outreach, shelter, housing and/or community-based programs in their defined area
  • Directly supervise the management level staff over these specific programs
  • Ensuring the programs maintain compliance with contract requirements
  • Ensuring programs meet productivity requirements
  • Ensuring programs achieve program defined outcomes
  • Responsible for the development of their assigned managers and staff including creating training plans to address trends in gaps of care
  • Working with managers on the review of innovative approach to service delivery and guiding them through implementation
  • Working in partnership with the CBI finance department on grant management and budget tracking
  • Monitoring for program compliance
  • Represent Community Bridges within the community, seeking out new partnerships and funding opportunities to expand the Community Bridges continuum and improve the system of care
  • Be the liaison for the CBI Patient Centered Medical Homes, Crisis, and Inpatient units streamlined connection to the Housing and Community Integration resources and knowledge
