About The Position

Our homecare teams are Interdisciplinary teams providing hospice services and support to patient and caregivers in their home environment. Each team is divided up to serve a specific geographical area. Each member of the Interdisciplinary team (Nurse, Social Worker, Chaplain, Nursing Assistant) provides discipline specific services within their scope of practice to their assigned patients, while working as a collaborative team to support our mission. The Assistant Team Manager is responsible for managing the day-to-day clinical activities and administrative functions of the office as assigned by the Clinical Team Manager, Regional Director or the Senior Director. Responsibilities include supervision of the Clinical Care Coordinators (CCC) and support of the Clinical Team Managers (TMs) and clinical staff. Establish schedule and assist in orientation of new staff in the field. Facilitates problem-solving discussions and/or symptom management consultations with clinical staff when assigned to cover the office responsibilities. Attends scheduled Nurses Meeting, department meetings and other relevant in-services to ensure integration of information to the staff. Oversees CCC performance and their supervision of Nursing Assistants and Homemakers. Serves as a clinical resource for clinical staff. Attends and assists in facilitation of IDT on as needed basis in absence of TMs or Director. Provides direct patient care as needed based on census and staff availability.

Requirements

  • Bachelors Degree in Nursing required. Or the equivalent combination of education, technical certifications, training, and work experience.
  • Minimum 2 years related experience required with increasing management responsibilities.
  • Related experience includes home health/hospice, oncology, and medical surgical.
  • Proficiency in Microsoft Office is required.
