About The Position

To be fully engaged in providing Quality/No Harm, Customer Experience, and Stewardship by managing the Clinical Supervisor team. Additional functions include participation in the development and implementation of policies and procedures for the Home Care department. Interview and selection of personnel, scheduling and assignment of personnel, input into employee performance appraisals, quality improvement, and meeting financial targets. Serving as clinical and professional role model to staff and contributing to their professional growth and development as well as fostering connections with customers both internal and external.

Requirements

  • BSN
  • Three (3) years Home Health nursing experience.
  • Two (2) years acute care experience as a RN.
  • Current RN Licensure in the State of Florida or endorsement.
  • Valid Driver's License and clean Motor Vehicle Record and maintained.
  • American Heart Association (AHA) Basic Life Support (BLS) Healthcare Provider Completion Card prior to start date and maintained.
  • Must satisfy Florida of Agency for Health Care Administration (AHCA) Level 2 background screening.
  • Oasis documentation.
  • CMS Conditions of Participation.
  • ACHA Home Care Standards.
  • TJC Standards.

Responsibilities

  • Promotes associate accountability and competency through leadership and management practice. Encourages associate growth and development through mentorship and goal setting.
  • Assists in providing consistent on-boarding experience for all new clinical staff.
  • Provides input for annual performance appraisals and competency checklists for personnel on an annual basis and as necessary in a timely manner.
  • Interviews and participates in the selection of new clinicians.
  • Assists with implementation of utilization-based performance improvement activities resulting in improved operations and patient care outcomes.
  • Identifies opportunities for quality improvement activities in the department.
  • Works with individual nursing supervisors and their clinical teams on opportunities to increase Patient Engagement scores, along with HHVBP incentives.
  • Ensures compliance with the Joint Commission and other organizational and regulatory agency standards with quality indicators and core measures with on-going education.
  • Monitors and ensures compliance with staffing guidelines, utilization and productivity to meet financial goals.
  • Communicates staff, equipment needs and concerns to Manager/Director as appropriate.
  • Define and communicate a clear, compelling vision for the team that effectively ties into the mission and vision of Health First, and inspirationally lead the team to achieve that vision.
  • Provide leadership, motivation, coaching, feedback and support to foster and strengthen growth and development of an effective, high performing team.
  • Lead change through effective communication, explaining the connection and value to the organization, creating stronger buy-in and urgency, while understanding impact to the team to obtain commitment.
  • Demonstrate openness to hearing diverse ideas and thoughts; create a sense of inclusivity; and encourage collaboration across teams to help break down silos to meet the team's and organization's goals.
