About The Position

Coordinates and provides family-centered nursing care to infants, children and adolescents in the home setting. Accountable for professional practice including independent nursing functions and delegated medical functions performed in collaboration with other health members. Responsible for continuous review of all aspects of every patient on his/her caseload to include\: appropriate utilization of services; ensuring continued skilled need; monitoring of homebound status; review of documentation in the medical record; maintenance of interdisciplinary communication and discharge planning.

Requirements

  • Current Minnesota RN License, required.
  • Current BLS Certification, required.
  • Must have a working automobile.
  • Mileage reimbursed based on federal rate.
  • Must possess a valid driver's license (must meet minimum criteria of Children's Minnesota Fleet Safety Management policy).
  • Clean driving record.
  • Must possess and provide verification of current automobile insurance coverage.
  • All job offers are contingent upon successful completion of an occupational health assessment, drug screen, background investigation, and compliance with the U.S. Government Form I-9, Employment Eligibility Verification.
