About The Position

Assists the Director to plan, organize, direct, and monitor Case Management Department day-to-day activities.

Requirements

  • Bachelor's Degree in Nursing (BSN) required.
  • Minimum three (3) years clinical experience in direct patient care.
  • Minimum one (1) year supervisory experience in quality assessment or utilization management.
  • Critical thinking, math, and computer skills.
  • Works as a team player.
  • Demonstrates respect and professionalism in all interactions.
  • Excellent oral and communication skills.
  • Decision making and problem solving.
  • Able to read, write, see and hear English.
  • Ability to fully utilize BHMC’s electronic record systems, equipment and other healthcare and billing systems relevant to this position.
  • Registered Nurse; licensed in New York State required.
  • Basic Life Support (BLS) required.
  • Advanced Cardiovascular Life Support (ACLS) required.
  • Physically demanding, high-stress environment requiring prolonged periods of standing, reaching, walking throughout the working day.
  • Position will be required to stoop, bend, lift, and carry items weighing up to 35 pounds for routine performance of essential functions.
  • An individual in this position may be exposed to: Adverse indoor environmental conditions, Blood borne pathogens, Blood and body fluids, Physical hazards (e.g., needle pricks, etc.), communicable diseases, chemical hazards, radiation, and repetitive motions.
  • Full range of body motion, able to lift up to 100 pounds, if necessary, to assist lifting and moving patients.
  • Manual and finger dexterity, as well as hand and eye coordination.
  • Corrected vision and hearing to within normal range.

Nice To Haves

  • Master's Degree in Nursing (MSN) preferred.
  • Case Manager Certification (CCM) preferred.
  • Pediatric Advanced Life Support (PALS) preferred.

Responsibilities

  • Supervises and monitors Case Management activities.
  • Conducts and coordinates Case Management Nurse’s orientation and training.
  • Develops Case Management Department policies and procedures.
  • Collaborates with other hospital departments.
  • Identifies, documents, and trends utilization issues; creates and facilitates implementation of solutions with appropriate staff/departments.
  • Serves as resource person to Case Management staff and other hospital personnel.
  • Represent the Director at meetings and in the Director’s absence.
  • Identifies problems taking appropriate measures to assure resolution.
  • Interprets and assures compliance with regulatory bodies.
  • Conducts other duties as requested by management to ensure efficient department and hospital operations.
  • Performs other related duties assigned by supervisor. In an emergency, performs other duties necessary for the welfare of patients or the efficient operations of the institution.
  • Other duties as assigned.
