About The Position

We’re looking for a compassionate, collaborative, and data-savvy clinical leader who thrives in a fast-paced, mission-centered, and team-based care environments. As Assistant Clinical Director, you’ll be reporting to the Market Medical Director and partnering together to support a care team that is dedicated to reimagining healthcare for patients with complex needs—those too often left behind by conventional healthcare systems. As the Assistant Clinical Director, you will: Deliver “top of license” care through direct patient visits across home settings in the community Guide and inspire a team of field-based advanced practice clinicians to elevate clinical quality, performance, and outcomes Use data and KPIs as tools to achieve real, measurable change Build trusted relationships with key clinical and community partners, including SNFs, hospitals, and home health providers Support onboarding, mentorship, and ongoing performance development for APP team members Support value-based care initiatives executed by cross-functional teams

Requirements

  • Certified Nurse Practitioner or Physician Assistant with active licensure and DEA credentials in the state of New York.
  • 3+ years of clinical experience with aging or medically complex populations
  • Proven leadership capabilities and experience managing clinical teams
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 3 5 years. No vehicle-related suspensions/reinstatements, DUI, reckless driving, or felony convictions within the last 7 years. Multiple violations and accidents within the last 5 years would be subject to review.

Nice To Haves

  • Public health, business administration, and/or health administration studies preferred.
  • Previous experience with managed care and/or value-based healthcare delivery preferred
