About The Position

This role is responsible for assessing an individual's needs and coordinating services to support their health, safety, and independence. This role conducts in-home and telephonic assessments across the eight-county service area to evaluate an individual’s eligibility for nursing home level of care services, Medicaid waiver programs, and state-funded services that support aging in place.

Requirements

  • Active LSW or RN Ohio licensure.
  • Valid driver’s license and reliable transportation with proof of insurance.

  • 1 year of experience in aging and disability services, case management, or healthcare assessments.
  • Experience conducting in-home assessments.
  • Knowledge of Medicaid waiver programs, long-term services and supports, and community-based care options.
  • Familiarity with PASSPORT, Assisted Living Waiver, and other Ohio Medicaid waiver programs.
  • Excellent time management skills to balance home visits and documentation timelines
  • Proficiency using electronic health records.
  • Accurately maintain case notes and client records.
  • Active listening with empathy and support to community members.
  • Cultural competencies to support clients from diverse backgrounds.
  • Knowledge of community resources.
  • Passionate about working with older adults and those with disabilities.

Responsibilities

  • Utilize the designated documentation system to complete all required assessments, including Level of Care, Term Consultations, PASSPORT, Assisted Living, and Residential State Supplement assessments.
  • Conduct comprehensive in-home assessments to determine nursing facility level of care eligibility for the programs, as well as other long-term services and supports.
  • Determine eligibility for state-funded and Medicaid waiver programs based on regulatory criteria.
  • Provide detailed information to consumers and caregivers regarding available programs, alternative community resources, and service options to support informed decision-making.
  • Ensure assessments are completed and documented in a timely and accurate manner in compliance with program timelines and requirements.
  • Collect and review collateral/supporting documentation as needed to verify information and complete assessments thoroughly.
  • Coordinate assessment scheduling to accommodate the needs and availability of consumers and their family members or caregivers.
  • Maintain ongoing knowledge of applicable state and federal program regulations, eligibility criteria, and available community resources.
  • Participate in team meetings, training, and professional development opportunities.
