About The Position

The role Assessor/Care Manager is to initiate the process of evaluating consumer needs and providing information regarding available options. This position’s primary duty is to perform work directly related to the successful operation of the Agency. Accordingly, the employee must exercise discretion and independent judgment with the respect to matters of significance. All staff have the responsibility and authority to propose ways to improve quality and to fully participate in quality improvement efforts. Perform any other job related instructions as requested, with reasonable accommodation.

Requirements

  • Current licensure as a social worker by the State of Ohio Counselor and Social Work Board, or as a Registered Nurse with the Ohio Board of Nursing.
  • One year of experience in home care, geriatrics, home health care, medical social work, or other substitute as approved by the Ohio Department of Aging.
  • Basic knowledge of major chronic illnesses, disease processes, normal aging, and basic knowledge of medical terminology.
  • Aptitude to develop care plans by identifying implication, goals, and interventions.
  • Excellent verbal and written interviewing and assessment skills.
  • Knowledge of local resources for older adults.
  • Ability to communicate in a collaborative, effective manner with others and maintain good working relationships.
  • Basic computer skills including Microsoft Office applications.
  • Potential to complete job duties by telecommunication from home.
  • Skilled to record information in an accurate, timely manner and meet program time frames.
  • Ability to use and transport a laptop computer and case, therefore must be able to lift/carry a minimum of twenty (20) pounds.
  • Capability to ambulate enough to access private homes/locations which may not be readily accessible.
  • Possession of a driver’s license with appropriate insurance coverage as required by agency standards.

Responsibilities

  • Conduct initial comprehensive face-to-face assessments in the community setting and/or facility settings
  • Meet compliance for assessments and contacts/visits as assigned per Ohio Department of Medicaid (ODM) requirements.
  • Provide information on MyCare waiver benefits.
  • Report productivity to supervisor/manager daily, weekly, and per request.
  • Advocate and educate in the best interest of the consumer/individual through collaboration on issues relating to goals and objectives of an established care plan.
  • Maintain accurate documentation of consumer/individual contacts within the system of record to meet state and federal reporting requirements
  • Meet compliance with quality assurance standards.
  • Develop care treatment plans and service plan authorizations.
  • Travel to the communities within the five county service area (Cuyahoga, Lorain, Geauga, Lake and Medina) to conduct initial assessments.
  • Maintain consumer confidentiality according to agency policies
  • Report abuse, neglect or exploitation of older adults as a mandated reporter as required by Ohio law.
  • Abide by the professional code of ethics established by licensure.
  • Participate in trainings, meetings, in-services, and continuing education opportunities as required by position and supervisor or manager
  • Perform any other job related instructions as requested, with reasonable accommodation.
