We are seeking a full-time Peer Support Specialist to join our Assertive Community Treatment (ACT) team. In this role, you will provide essential expertise and consultation to promote a culture that respects and integrates each client’s preferences, values, and unique perspectives into their treatment plans. You will support clients in achieving symptom stability, improving their quality of life, and fostering independence within their communities. Service Coordination: Collaborate with the ACT team leader to monitor clients’ clinical status and responses to treatment. Assist in identifying, planning, scheduling, and implementing services tailored to each client’s needs. Client Advocacy and Support: Act as an advocate for clients in the community, supporting their access to necessary resources and opportunities. Assist in the development of transition plans when clients move in or out of services. Documentation and Compliance: Maintain accurate records, including treatment notes, goals, and input into person-centered plans. Ensure adherence to the ACT model, as measured by TMACT standards. Team Collaboration: Provide consultation to the ACT team to promote a person-centered approach to care. Support a culture of respect and understanding for clients’ points of view and preferences. Availability: Be available to respond to clients and co-workers as needed, including 24/7/365 on-call support. This description outlines the general scope and level of work for this role.
