Full time NP or PA opportunity as the Thoracic Oncology Service Line Coordinator. Day shift/ clinic hours with some remote opportunities. Job duties include: Point of contact for the oncology program patients and providers Receive and review internal and external referrals. Perform initial consult visit on all cancer patients. Order and schedule appropriate testing for ongoing cancer follow-up or staging. Collaborates with interdisciplinary team to ensure appropriate and efficient treatment plans are in place. Presents and coordinates tumor board patient presentations. Provide education and support through the cancer journey including but not limited to, assistance in understanding diagnosis, treatment options, and medication regimens. Facilitates and streamlines communication across the care continuum Work across healthcare disciplines to ensure timely appointments, results reporting and communication. Identify potential barriers to care. Development of patient education materials, clinical pathways, and quality studies. Participate in diagnosis related clinical trials. Promotes wellness and recommends community resources to meet patient and family needs. Knowledgeable of reimbursement procedures and documentation requirements. Work with cancer registry staff and others to collect data, track outcomes and support strategic planning processes. Consistently deliver the best in customer service and patient care.
