LOCATION: Swainsboro, Georgia  The Community Service Board of Middle Georgia's Child and Adolescent (C&A) program is seeking a qualified individual to serve as our next Apex CSI Case Manager. This school-based case manager will be responsible for handling cases related to delivery of mental health care to assigned clients at the designated school locations and will provide resources to parents and the school systems for individual patient needs. Please note, travel to and from schools is required.

Requirements

  • High School Diploma/GED Equivalent
  • Valid Georgia Driver’s License
  • Effective verbal and written communication skills
  • Strong interpersonal skills and the ability to work effectively with diverse communities
  • Ability to work independently and in collaboration with others
  • Experience with Microsoft 365 Office Products
  • Ability to organize, prioritize and meet deadlines accordingly

Nice To Haves

  • Bachelor’s degree from an accredited college or university in the field of psychology, social work, counseling education, human services or a related field of study
  • Previous case management experience

Responsibilities

  • Providing a broad range of social services to individuals served in Emanuel County Schools and their families
  • Ensuring individuals go to their scheduled doctor appointments and take medication as prescribed
  • Contacts consumers parents, teachers, and school counselors on a weekly basis and reporting findings to supervisor and MD, ensuring all progress notes are correct
  • Completing progress notes, no later than 24 hours after delivering the service, having said progress notes scanned and shown in the EMR no later than 7 days after providing the services, and checking EMR weekly to ensure that progress notes are showing in EMR
  • Working with scheduler in completing weekly schedule to ensure monthly contacts are met and maintained
  • Communicating regularly with staff on progress toward defined goals and/or required results, providing specific feedback, and initiating corrective action when defined goals and/or required results are not met
  • All other duties as assigned
