Make a Difference Community Heart and Vascular has a part-time Ambulatory Triage LPN position available at the Cardiac Clinic. The Ambulatory Triage LPN is responsible for addressing patient's condition/symptoms virtually, giving appropriate clinical direction, education, and recommendation(s) for disposition.

Requirements

• Graduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience.
• Licensed as a Licensed Practical Nurse (LPN) by the Indiana Professional Licensing Agency (IPLA).
• One to two years of ambulatory and/or triage experience in a clinical setting required.
• Possesses knowledge of necessary computer systems including Microsoft Office, Outlook, and electronic medical records (EMR).
• Supports the mission, vision and PRIIDE values of Community Health Network.

Nice To Haves

• Cardiology experience preferred.

Responsibilities

• The Ambulatory Triage LPN is responsible for addressing patient's condition/symptoms virtually, giving appropriate clinical direction, education, and recommendation(s) for disposition.
• Utilizes critical thinking skills, education, mentoring, guidance, and interventions to achieve goals of care virtually.
