Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations. URMC’s Forensic ACT team, will serve justice-involved adults with serious mental illness in Monroe County. The Project FACT team nurse provides overview of physical and behavioral health issues of individuals receiving services from the team. The nurse provides leadership and guidance to the team in addressing all preventative, ongoing and critical health matters. The nurse also coordinates pharmacological services along with the team’s prescriber(s). Nurses are core members of the team and contribute to all clinical decisions in team, including treatment plans and documentation. Rotating on call responsibilities. A university car is provided for use in seeing clients. Requirements

  • NYS RN license in good standing.
  • Must possess a valid New York State Driver's license and a satisfactory driving record.
  • Minimum 3 years of Psychiatric experience required, outpatient experience preferred.

Responsibilities

  • Under the direction of leadership, is accountable for provision of competent direct patient care based on established standards.
  • Contributes to unit activities and supports enhancement of the care delivery system at the unit level.
  • Supports peers in the delivery of patient care services and assumes an expanded role which may include charge nurse, preceptor, and committee member responsibilities.
  • The Project FACT team nurse provides overview of physical and behavioral health issues of individuals receiving services from the team.
  • The nurse provides leadership and guidance to the team in addressing all preventative, ongoing and critical health matters.
