Come join our team as a full-time Certified Diabetes Educator with BJC Medical Group located in St. Peters, this is a rare opportunity to join a great team! Role Purpose Provides Diabetes and Nutritional educational services and community resources for inpatients and outpatients. Maintains a case load of patients and promotes program development.

Responsibilities
- Monitors patients response and modifies plan of care accordingly.
- Provides accurate documentation as required.
- Delivers direct patient care by assessing and evaluating patient's physical, psychosocial and emotional needs.
- Develops and implements individual plans of care in accordance with the established guidelines and standards of nursing care.
- Educates patients and their families on how to manage their condition.
